Information for patients visiting our clinic for the first time

Thank you for choosing Sukoyaka Kodomo Clinic Ukima.
To help you and your child feel at ease, this page explains what to expect on your first visit.

Making an Appointment
Online Booking (Recommended)
You can book anytime, 24 hours a day.
First-time patients can use the same system.
The booking website is in Japanese.
What to Bring
Clinic Patient Card
My Number Card
Please bring your child's My Number Card.
Please check that it has not expired.
Medical Subsidy Certificate
Please bring your child's medical subsidy certificate
(such as the infant or child medical certificate, "Iryo-sho").
Medication Record Book
If your child is currently taking any medication,
please bring the medication record book ("Okusuri Techo").
Maternal and Child Health Handbook
Please bring the Maternal and Child Health Handbook
("Boshi Techo") so we can check vaccination records and other details.
Referral Letter
If you have a referral letter or test results from another clinic or hospital,
please bring them.
Online Questionnaire
Filling out the online questionnaire in advance
helps your visit go smoothly.
On the Day of Your Visit
Reception
Check-in is done with the My Number Card.
In case face recognition does not work, please make sure you know the card's PIN.
If you have already completed the online questionnaire, please let the reception staff know.
Questionnaire
If you have not completed the online questionnaire, please fill it out online or on a paper form at the clinic.
Please describe your child's symptoms and any concerns in detail.
Waiting Room
Please wait in the waiting room until your child's name is called.
Picture books and other items are available.
Examination
The doctor will examine your child carefully.
Please feel free to ask about anything that concerns you.
Payment and Next Appointment
If needed, we will book your next appointment.
If you receive a prescription, please collect the medicine at a nearby pharmacy.

To shorten waiting times, we recommend filling out the online questionnaire in advance.
You can easily complete it on your smartphone or computer before coming to the clinic.
The questionnaire is in Japanese.
